This website is for Dr. Nitish Mandal, Consultant Pain Physician. The clinics are Koramangala Pain Clinic and J.P. Nagar Pain Clinic.

If you browse the site, request an appointment, fill in the pain assessment, save a profile, or write to the clinic, this policy describes that information.

You choose what to send. The forms ask only for what the clinic needs to arrange a visit or read before you arrive.

• Appointment request: first name, last name, sex, age, email, phone number, pincode, how you heard about the clinic, where the pain is, how long it has been present, preferred clinic, date, time, and any message.
• Pain assessment: name, age, gender, occupation, pincode, phone number, how you found the clinic, and your answers about pain, sleep, family life, work, past care, and your treatment goal.
• Profile, if you sign in: the phone number or email used for a one-time code, contact details, symptoms, notes you type, and past treatments you add.
• Calls, email, and WhatsApp: whatever you send when you contact the clinic that way.

The site is hosted on Vercel. Hosting logs can include your IP address, browser type, and the pages requested. Those logs are used to run and protect the site.

This website does not use a separate advertising or analytics tracker.

If you create a profile or sign in, Clerk sends the one-time code to your phone or email and confirms it. Clerk stores the sign-in account. The clinic stores the profile details you save after that.

Appointment details are used to hold a requested time and to reach you if reception needs to confirm.

Assessment answers and profile notes are used so the clinic can read what you typed before the visit.

Contact details are used to reply to you.

The clinic does not sell personal information and does not use it for advertising.

Appointment requests are stored in the clinic database when that database is connected. A copy may also be written to a private Google Sheet, added to Google Calendar, and emailed to the clinic.

Pain assessment answers are stored for the clinic and, when configured, copied to the same private Google Sheet.

Profile details are stored in the clinic database and linked to your sign-in account.

If you tap a WhatsApp link, that message is sent through WhatsApp.

What you type on this website is information you chose to share before a visit. The clinic medical file is kept at the clinic. Diagnoses and prescriptions are not stored in the website profile.

Dr. Nitish Mandal and clinic staff who arrange visits can see what you submit.

Service providers who host the website, store the database, handle sign-in, or deliver the email, calendar, and spreadsheet copies of a booking can process that information for those tasks.

The website does not send your details to insurers or to other doctors. You can ask the clinic to share records with someone else.

Booking and assessment records are kept for as long as the clinic needs them to arrange care, answer a follow-up, and meet clinical and legal record duties.

You can ask the clinic to correct or delete profile details on this website when those details are not required to be kept.

You can request a visit by phone or WhatsApp without creating a profile. Signing in is optional.

To ask for a copy of information you submitted on this website, or to ask for a correction or deletion, email teamdrnitish@gmail.com.

You can sign out of a profile at any time. Signing out does not delete records the clinic must keep for a visit that was already requested.

Use this website for your own care, or for a family member when you are arranging their visit. Submit a child's details only if you are the parent or guardian arranging that visit.
