Chest Pain but Your ECG is Normal? Why It Might Be Costochondritis

It is a terrifying moment: a sharp, stabbing pain in your chest that radiates with every breath. You rush to the ER, undergo multiple ECGs, and even a TMT (Treadmill Test), only to be told, "Your heart is perfectly fine."
While "normal" results are a relief, they don’t stop the pain. If your cardiac tests are clear but your chest still hurts when you move or touch your breastbone, you may be dealing with Costochondritis.
What is Costochondritis?
Costochondritis is the inflammation of the cartilage that connects your ribs to the sternum (breastbone). Because the pain occurs in the chest wall, it often mimics the symptoms of a heart attack, leading to significant anxiety.
Why Do Standard Cardiac Tests Miss It?
Tests like an ECG (Electrocardiogram) and TMT are designed to measure the electrical activity and stress response of the heart muscle. Since Costochondritis is a structural, inflammatory issue of the rib cage and not a cardiac event, these tests will almost always return normal results.
Common symptoms include:
Sharp, aching, or pressure-like pain on the left side of the breastbone.
Pain that worsens when taking a deep breath, coughing, or physical activity.
Tenderness when pressing directly on the rib joints.
The Path to Relief: Diagnostic Blocks in chest pain
When traditional painkillers and rest fail, we look for a definitive diagnosis. This is achieved through a Diagnostic Block.
By injecting a small amount of numbing agent into the specific intercostal space or the affected costochondral joint, we can "turn off" the pain signals temporarily. If your pain vanishes instantly after the block, we have confirmed that the source is the chest wall and not an internal organ.
Targeted Injections: Long-Term Recovery
Once diagnosed, a Targeted Interventional Injection can provide lasting relief. Using precise guidance, a combination of anti-inflammatory medication is delivered directly to the inflamed cartilage.
The benefits of targeted treatment include:
Immediate Pain Reduction: Minimizes the localized inflammation.
Avoidance of Oral Meds: Reduces the need for long-term painkillers that can affect the stomach or kidneys.
Restored Mobility: Allows patients to return to their daily routines and exercise without chest wall restriction.
